Pre-participation Screenings for High School Athletics

Pre-Participation Sports Screening Assessments


 


 


Purpose: The SDSMF is pleased to provide pre-participation sports screening for all High Schools Students in SUHSD. Each student is required by CIF to have a medical screening before they can participate in sports or activities each school-year. The SDSMF will provide medical personnel and volunteers to conduct the screenings on specific dates. All nine high schools in the district will be assigned to a date. The screenings will take place in the gymnasium of one the high schools assigned to a particular date.

 

Each student who participates in the pre-participation screening will have a completed Medical Form valid for one year to participate in sports or activities. Each Medical Form will be signed by a medical doctor and verified by a stamp from SDSMF.
 
Criteria: Each student requesting a screening must be enrolled in a high school in the SUHSD. Each student must provide a signed Parent Consent form at check-in for the screening.

Cost: $20.00 per student. Paid to the SDSMF via cash ONLY for the screening.

Goal: A percentage of the money collected from the Sports/ Co-curricular Participation Screening Risk Assessment Program dates will be divided evenly and donated back to each school's Athletic Department training room budget. The total amount donated will be less the hard costs to conduct the pre-participation screenings. We are estimating 90% to be returned to the participating schools. These goals are based on student attendance. Our budget is around $8,000 for the entire program. We need to examine about 800 to 1000 students’ district wide to be able to donate back an estimated amount of $800 to $1000 per school! In 2010 we had 800 student atheletes pariticpate in the sports assessments.
